Lost/Stolen Passports Containing Valid U.S. Visas

If your passport containing a U.S. visa is lost or stolen, please report this right away to the Embassy.  The steps you should take are:  

  1. File a report with the police in the jurisdiction where the passport was lost or stolen.
  2. Send a copy of the police report, along with the following information, to the Consular Fraud Prevention Unit of the U.S. Embassy in Colombia:
     - Last names and first names as they appeared on your passport.  (PLEASE    WRITE IN CAPITAL LETTERS)
     - Date of birth, cédula number.
     - Whether the passport was lost or stolen, and how.
     - Date of loss.
     - Your phone and e-mail contacts.

By e-mail: FPMBogota@state.gov  (Scan the police report and attach it to the e-mail.)

At that point, your visa will be invalid for travel and you must apply for a new visa if you wish to travel to the United States.

If you recover the lost passport with a valid U.S. visa, report the recovery to the Consular Fraud Prevention Unit.  The Embassy will then reactivate your visa, confirm the reactivation with you, and you will not need to apply for a new one.
